What You Need

If you submit a claim by fax, be sure to include this information in your fax:

Your policy number
The date the loss happened
The policyholder’s name and address
A description of what happened
If possible, please include a telephone number where we can contact you

 When you fill out and submit our online claim form,  we’ll document the information on our system.  A member of our claim staff will contact you to let you know what will happen next.
